What is Pharmaceutical Waste?

Pharmaceutical waste refers to expired, contaminated and unused medicines (or leftover medications)—including prescription drugs, over-the-counter medications, and controlled substances—that must be safely and compliantly disposed of to protect healthcare workers, patients, the public, and the environment. In Canada, this waste is strictly regulated and must be managed according to federal and provincial environmental and health regulations.


Types of Pharmaceutical Waste

Beyond standard pharmaceutical waste (generally defined as any expired, unused or leftover medication), it’s important to know and understand these additional types of pharmaceutical waste. 

  • Controlled Substances: Defined under the Controlled Drugs and Substances Act (CDSA), these include prescription medications such as narcotics, opioids, and stimulants.
    • May be hazardous or non-hazardous
    • Must be stored and destroyed using secure, compliant solutions (e.g. Rx Destroyer, Secure a Drug)
    • Require chain-of-custody tracking and witnessed destruction
  • Cytotoxic Sharps Waste: This waste stream comprises anything capable of causing punctures or cuts that has come in contact with cytotoxic medication. This waste must be placed into a red cytotoxic sharps container, such as Chemosmart.
  • Non-Sharps Cytotoxic Waste : Any items (other than sharps) that have come into contact with cytotoxic drugs as well as bodily fluids of patients receiving cytotoxic medication within the previous 48 hours. Examples include cytotoxic drugs and packaging, IV bags, tubing, gloves and gowns
